Identify the document, incident reference, date and location you need. Records Unit assesses each request and explains any withholding or redaction. A request does not automatically grant access to every department record.

Patient care records require additional identity and authority checks. Access to an incident summary does not grant access to a patient’s clinical narrative. Protected information about other people may be removed.
